pthreadVC.dll is needed by gem.dll, that is because gem is still compiled with MSVC, pd-extended package includes libpthread-2.dll, from mingw binaries, so you need to get pthreadVC.dll, at least it should be provided by gem package.

I tried it once again, deleted gem 0.92.3, installed 0.93.1, get the error message, complete reinstallation of pd-ext 0.42.5, then install gem 0.93.1 again, same error message. Perhaps someone could make a zip file and send me that pthreadVC.dll and tell me where to place it, would it go in the gem folder? Program files (x86)/pd/extra/Gem?

afaik, pthreadVC.dll comes with all binary releases of pd-vanilla.

The link is no longer working, you'll have to search for the file by downloading the latest pd-vanilla, looking in the bin/ folder in program files copying them to Program Files (x86)/pd/bin/ folder for pd-extented.
