JACK is system for handling real-time, low latency audio (and MIDI). It runs on GNU/Linux, Solaris, FreeBSD, OS X and Windows (and can be ported to other POSIX-conformant platforms).

I forgot one thing, careful of buffer size (in pd it should be twice jack size) and samplerate settings (must e the same), and run jack (drivers: portaudio in qjackctl.exe settings) before starting pd.exe, have fun again, :)
