Actually, if pd open did take an optional argument, this would be easy to handle ala:
All other calls to pd open should omit the extra argument and behave as before. I'd see this as a simple improvement for new users.

actually, it's probably not a serious problem that one can multiply open help files (if one really wants to), so probably it's not worth fixing this.
On the other hand, a naive user on a Mac would expect that clicking on a file in the "finder", if Pd already has the file open, would show the user the open file instead of opening another copy.
Supposing the "open" menu called "pd open" with the third nonzero argument, but if "pd open" acted as it does now so that one could programmatically open multiple copies of a patch, would this permit you to do what you're planning? (I think that this would be patch-level back compatible).

I think there is no consistent cross-platform behavior in this regard. While on MacOS this will open in the same Pd instance, Linux will open another Pd to open the file with (Windows too IIRC).

at least with the Debian packages, Pd will open the patch in an already running instance (if possible). (other distros might have the file-associations wrong; if you install Pd from source, there are not file-associations at all).
on Windows, there is a bug-fix coming with Pd-0.49, so that patches opened via the file-associations will open in an already running instance as well.

Do you mean the file associations in the File Manager? If not, is it possible for the user to make these file associations? For the File Manager, in Dolphin "pd" is the application associated with the file which is the one in ~/.local/share/applications (currently Pd-0.49-0test5) which I have installed from source.

i mean the file associations via mime-types, which are (normally) also picked up by your File Manager.
If not, is it possible for the user to make these file associations? For the File Manager, in Dolphin "pd" is the application associated with the file
the file-association should be for "pd-gui" rather than "pd" (the Pd-GUI has all the logic to prevent double starts; the Pd-core does not, and imho should not)
