Ok,
I have some homebrewn externals that take an objects x,y and z position and the listeners position and orientation. The output of the external are values that can be multiplied to an audio signal to generate constant power panning levels for different speaker setups (4, 5.1 and 8 speakers).
You could also look at the vbap external, which also can do 3D stuff.

...or at rvbap (= reverberated vbap) which is currently only available for Pd on IRIX (http://www.nullmedium.de/dev/irix/) or Max/MSP (http://www.akustische-kunst.org/maxmsp/). Once I get all the bugs fixed it'll be available for Pd on OS X and Windows as well.
