Using pd-38 or newer you can set startup flags in the "preferences" menu under "startup" there is a feild for "startup" where you just specify any flags you want. Though for MIDI I imagine all the audio settings do is set this flags anyhow.

well, even if that were the case, it wouldn't be that big of a deal. pc final scratch pro only works on linux too, right? ;)
[midiin] is the raw midi interface.
you do have notein/noteout & ctlin/ctlout on windows as well as a host of other midi-related objects that work just fine.
