Reminds me of a bug, that the .pd extension is not added to pd patchnames when saved on OSX. On other platforms (at least linux) the .pd is added automatically...
where is that PD tracker again...

Maybe you aren't use the File extensions? If you save a patch, make sure it ends in ".pd", otherwise Pd will not accept it. Also don't use names with funny characters like umlauts or spaces.
You also could send us the file you cannot open.
