Thanks for the thoughtful replies - last night I got to grips with mrpeach's midifile and noteout and was able to get things working, it looks as though it will do the kinds of things I am after.
I am now using midifile to read and play the midi file, driving it with a metro, and tweaking its output on the way to noteout. I haven't tried controllers yet, but quite possibly they will work in a similar manner.
So I guess that [midiout] is broken?
For the record, seq-test.pd and seq-help.pd are in the pd-extended distribution, and both don't work for me (both use [midiout].
Thanks all for the suggestions & advice! Rua HM.

Thanks for the link, although that just looks to be a video of someone playing with the help patch :)
I think, the [seq] object from Cyclone follows the Max seq object, so you could look in the Max documentation from cycling74.com how it works. AFAIK there is no Pd help patch.
Oh, well, obviously in the video someone wrote a help file. ;)
The URL is in the video, so just download it from http://jkant.altervista.org/xfiles/seqdoc.zip
