libpd does (handle instantiating patches) as of about 2 weeks ago. It maintains the $0 value of the patch as well, so you can send values to unique receivers. You can make as many instances of a patch as you want and control them all separately, on the fly.
This is still only possible with the "usual" mechanism Pd offers, i.e. dynamic patching, and it has the same disadvantages.
Maybe LuaAV would be worth a look, too?
