Get ready to laugh... when I need to run gdb on an extern, I just link the thing into Pd, debug it, and then take it back out. Easier than figuring out how to get gdb to see dlopen()-ed code...

hm ... you are right, break file:nr also doesn't work for me ... I just read somewhere that maybe you can set "break main" and then when the program stops set the breakpoints for the libraries ... but I don't know ...
What platform are you on? I am on Mac OS X.
