I can recommend Wufoo (wufoo.com)! We use them often. They're friends of mine.
shouldn't it be possible to see all you are asking from the log files?
I think a lot of pages could be removed, because they are old (and/or
not used).
and the problem is not to criticize, but to find someone who wants to do
the work on the page.
