Hey
Could it be there is something wrong with the implementation of [midiout]? I am using ubuntu linux, with 0.40-3 extended. I have input from [cyclone/seq] which prints out fine, but the [midiout] object does not seem to react on this. I use alsa-midi, btw. With a 4 port output device. I have made midi files for each separate port, since midiout does seem to expect that, but that did not help one bit.

For some reason, I want to say that [midiout] isn't fully there. I
seem to recall advice to use [noteout] over [midiout], but that's all
I remember.

Yep, I agree, I think midiout is not working on linux and possibly windows too. I did see it working on Mac. On linux I was using alsa midi, but just soft out (via jack) into a softsynth.
Here's a mail to the list about it. http://lists.puredata.info/pipermail/pd-list/2008-11/066180.html
Sorry, I don't have any suggestions for how to fix it..
