mmm... i had some problem with fullscreen too (the command just maximize the window and do not make a "real" fullscreen) though i never really deeply looked into it as the maximised window is enough for me.
same for the screen selection, whatever i try in my xf86 (extended, dual head ..) i can't ask pdp to create a window elsewhere than in the screen pd is running, but same again, moving the window is an easy workaround so ...
for these two problems, you can use the "posdim x y w h" message ::
"posdim 0 0 1024 768" should get you to a real full screen mode.
"posdim 1024 0 1024 768" should set an xv window occupying the second display of your dual-head configured displays.

if you graphic card supports it yes, just duplicate pdp_xv and see if you can have several opened at the same time. if not, then also try to do the same with the gl output (pdp_glx). you can also have pdp_xv and pdp_glx running at the same time. i usually open glx windows for previews/things/whatever and have a maximized xv on the second screen. i prefer xv as a final output cause on my laptop for some unknown reasons the opengl doesn't sync correctly to the vertical refresh rate (even when it's supposed to do so).
