Probably the simplest way under linux is to use Fons Adriaensen's excellent zita-a2jbridge (http://kokkinizita.linuxaudio.org/linuxaudio/zita-ajbridge-doc/quickguide.html) to expose the additional cards as outputs under jack. This will remove the need for PD to do any kind of resampling or syncing.
