I believe that's what the external [vst~] was supposed to do - make your pd patch into a vst module usable in other programs.  I am not sure what the state of that one is though.. maybe someone more knowledgeable can chime in.

I don't know of anything like jack for Win32 (I assume this is what you mean, since OS X is unix-based too).  Sorry!
