If someone wants to get Pd-extended on Windows using Jack, I'll
contribute where I can. I don't think it would be a huge project,
mostly probably build system tweaks.

According to the JACK site it does!
JACK is system for handling real-time, low latency audio (and
MIDI). It runs on GNU/Linux, Solaris, FreeBSD, OS X and Windows
(and can be ported to other POSIX-conformant platforms).D.

If nature has made any one thing less susceptible than all others of
exclusive property, it is the action of the thinking power called an
idea, which an individual may exclusively possess as long as he keeps
it to himself; but the moment it is divulged, it forces itself into
the possession of everyone, and the receiver cannot dispossess himself
of it. - Thomas Jefferson
